As a new patient you will be contacted by our New Patient Coordinator. Your insurance information will be verified and our participation with your insurance company will be confirmed. We participate with most major insurance companies and will submit claims on your behalf. We do not accept Workman’s Compensation.
You will be asked to complete a patient information form prior to your appointment. Please be sure to have your valid insurance and personal identification cards with you at the time of your appointment. A photocopy will be placed in your billing file. Please be assured that your confidentiality will be respected at all times.
If you are covered by more than one insurance company, please let us know which company is the primary carrier. Failure to do so could delay payment of your insurance claim. If there are any changes to your insurance coverage at any time, please contact our office so that we may update your records.
Medicare patients without a supplemental insurance policy will be responsible for 20% coinsurance, payable at the time of service.
Copayments are payable at the time of your visit. We are unable to bill you for your copayment.
We accept cash, check, Visa, and Mastercard.
